Fat Grafting, also known as fat transfer is better than fillers. Fat grafting describes the procedure where fat taken from one area of the body and is introduced into another such that it gets enough blood supply in its new location to survive. Its popularity is increasing because it provides the volume and contour of youth, but unlike fillers, also slows the aging process and dramatically improves skin quality.

Fat Transfer to perform a butt lift can be done without implants. Buttocks augmentation also is quite successful; however, there are some caveats. First of all, there must be an ample amount of donor fat such that augmenting the buttocks does not create contour issues or disharmonies elsewhere. Second, buttock augmentation with fat grafting requires placing a large volume of fat into the buttocks subcutaneous layer. This layer typically does not have excellent blood supply so grafting while successful, is not as assured as in others areas. Placing fat into the buttock muscle, which has excellent blood supply, simply doesn’t give the volume expansion or shape that subcutaneous injecting does. Though I don’t use buttock implants because of specific issues that arise post operatively, I do use dermis-fat flaps from the body (back/hips/abdomen) as “buttock implants” and then augment those with fat grafting.

Fat Grafting can dramatically improve the overall result of a liposuction defect or cellulite. As a pioneer in the area of fat grafting, I have found that when fat is used to repair a liposuction defect it lasts for a very long time (if not forever.) The body’s natural reaction is to maintain the replacement for what was missing. The skin is smoother and the underlying fat provides additional antigravity support. Cellulite depressions can ruin an otherwise beautiful body contour. Release and grafting is a reliable adjunct to body contouring.

There are many misconceptions about Fat Grafting. Fat grafting is not as new a technique as you might think. It has been used for many years successfully and safely all over the world. In addition, contrary to other cosmetic enhancements, fat grafting is really not one procedure. It is highly technique dependent. . Some practitioners use fat grafting as filler, done under local anesthesia, with no down time but occasionally some bruising. The rejection of the damaged fat cells creates swelling that mimics fat transfer but is not the same procedure as surgical intervention with fat. It is curious that while other procedures such as rhinoplasty and breast augmentations are not considered generic, that the public does not give fat grafting and its best practitioners the same standing. It is a rapidly evolving field within plastic surgery worldwide and a specialized focus.
